- Individualized planning time with faculty.
- Take-home resources for hospital and facility presentations, orientation, and in-service sessions.

WHO SHOULD ATTEND:
We urge hospitals and health care systems to send an interdisciplinary team of participants that includes administrative leaders, physicians, nurses and other clinical staff, as well as patient and family leaders whose primary association with the hospital is as consumers of health services. This team also may include health care design professionals and other consultants working within the field.
SEMINAR RESOURCE NOTEBOOK:
All participants receive a Hospitals and Communities Moving Forward with Patient- and Family-Centered Care resource notebook and CD-ROM that provide useful, practical materials on patient- and family-centered care to facilitate the process of change in hospitals, long-term care facilities, and other health care entities within health care systems. The materials in these resources allow seminar attendees to immediately begin advancing and implementing the practice of patient- and family-centered care at their own facilities immediately upon return from the event.
